Two violations: shelving in walk-in coolers in need of replacement due to rust and cooler fans in need of cleaning and deep cleaning needed in produce area. Reinspection scheduled for Nov. 5.

10/40 Coffee, 14 Court Square, West Plains, preopening inspection for new location conducted on Aug. 7. Two violations: prep area cabinets in need of painting or sealing for ease of cleaning and no paper towels at prep area hand sink. The business committed to painting the cabinets and the paper towels were resolved onsite.

Alligator Express, 4714 U.S. 160, West Plains, inspected June 17. Three violations: pizza kept at 120 degrees and biscuits at 107 degrees in warmers, below minimum temperature of 135 degrees; thermometers not available at all prep coolers and food warmers; and outside dumpster not on a concrete pad. The first two items were corrected onsite, and the business committed to correcting the dumpster issue.

Arby’s 1342 Southern Hills, West Plains, inspected July 19. Two violations: prep cooler cutting boards in need of replacement due to scarring and pitting making them harder to clean and sanitize, and cobwebs in back storage area in need of cleaning. The business committed to ordering new cutting boards and corrected the cobwebs onsite.

Burger King, 1317 Preacher Roe Blvd., West Plains, inspected Aug. 2. In-depth cleaning must be performed prior to follow-up inspection. If corrections are not made, a work order will be issued. Reinspection date set for Sept. 1. Four violations: hair restrains not worn by all food prep staff; excessive buildup of food debris and grease observed on equipment, vent hoods, floors throughout the building and food contact surfaces in need of deep cleaning; cleaning schedule needs to be established and followed; and food waste stored in open buckets near hand washing sink in prep area.

Five violations: macaroni and cheese observed for sale past labeled use on time stamp in need of discarding; hot water at prep area sink measured at 95 degrees, below the minimum 100; three-vat sink had a leak in need of repair; no paper towels at sandwich prep area hand sink; and cleaning needed for prep area walk-in freezer and cooler flooring, bakery and pizza oven, vent hood and the counter under the soda fountain. The first item was corrected onsite; all others were given a deadline of Aug. 14 to be addressed.

Casey’s General Store No. 2032, 513 N. Elm St., Mnt. View, inspected June 11. Three violations: food boxes stored on floor in walk-in freezer, mold buildup in mop sink in need of cleaning and sanitizing, and no paper towels at prep area hand sink. The first and last of the items were corrected onsite, and the second, the business committed to addressing as soon as possible.

Dev’s Piano Bear and Lounge, 36 Court Square, West Plains, inspected July 19. Three violations: wiping cloths not stored in sanitizer when not in use; storage room had wood and sheetrock exposed in need of painting for ease of cleaning; and paper towels were needed at all prep, server and bar area hand sinks. The first and last items were corrected onsite and the second item must be corrected by the next routine inspection.

Diamond Head, 1214 Porter Wagoner Blvd., follow-up inspection conducted July 12. Previous violations were corrected. One new violation: Salad bar cold items were not held at 41 degrees or lower. Staff were instructed to keep additional ice in the salad bar to ensure cold holding, and to monitor temperatures.

Icon Asian Grille and Bubble Tea, 312 Porter Wagoner Blvd., Suite 200, preopening inspection conducted June 3. Two violations: wood/drywall in need of painting or sealing for ease of cleaning and no backflow prevention device at prep area mop sink. Reinspection was scheduled for July 3 and conducted July 19. Both items were resolved. One new violation: clean utensils stored on cloth towel instead of nonabsorbent surface. Reinspection was scheduled for Aug. 18 and conducted Aug. 8. Three violations: wiping cloths not stored in sanitizer when not in use, clean blender parts stored on cloth towel to dry instead of on nonabsorbent surface and dirty dishes were stored in the employee hand sink, making it inaccessible. All three were corrected onsite.

Jason’s Mexican Restaurant, 108 Main St., Willow Springs, inspected July 15. Two violations: chest freezer with excess ice in need of defrosting and no paper towels or dispenser at prep area hand sink. The business committed to defrosting the freezer at the close of business that day and corrected the paper towel issue onsite.

JB’s Healthmart, 1504 Preacher Roe Blvd., West Plains, inspected July 19. No violations.

Jimmie’s Family Restaurant, 805 Porter Wagoner Blvd., West Plains, follow-up inspection conducted July 11. All previous required violations corrected. One violation: flooring and walls in prep and dish areas in need of continued repair to make easily cleanable. A deadline was given to complete by next routine inspection.

Johnny Hatchets, 47 Court Square, West Plains, inspected Aug. 2. One violation: hot water faucet on prep area hand sink leaking and in need of repair. Business committed to correcting ASAP.

KC’s Sports Bistro, 409 Washington Ave., West Plains, follow-up inspection conducted July 12. Previous violations corrected. No new violations. Business encouraged to continue monitoring salad bar temperatures and ice levels, making sure to keep ice level with food to maintain proper cold temperature.

Kentzio Japanese Steakhouse, 1633 Bruce Smith Parkway, West Plains, follow-up inspection conducted Aug. 1. Two violations: two-door cooler registering temperature of 48 degrees, above the maximum of 41 degrees, and both back doors were prepped open, allowing the possibility of pest and bug entrance. The business committed to correcting the cooler temperature and discontinuing its use until done, and the doors were closed while onsite.

Little Caesar’s Pizza, 316 Porter Wagoner Blvd., West Plains, follow-up inspection conducted July 12. Will reinspect in October. Three violations: spray arm at three-vat sink below flood rim in need of being raised above the rim to prevent possible backflip, paper towel dispenser in dish area not working and excessive flies throughout the establishment. The first item must be corrected by the next inspection and the second was completed onsite. Business was instructed to use food-grade fly spray, fly lights and other food-safe measures to reduce or eliminate flies and a follow-up visit was planned in two weeks. The reinspection date is Oct. 7.

Los Amigos Mexican Grill, 1449 Preacher Roe Blvd., West Plains, follow-up inspection conducted Aug. 1. All previous violations corrected. Two violations: food product on floor in walk-in freezer and ice scoop laying in drink ice in front soda fountain ice bin. Both items corrected onsite.

Our Place mobile food truck, Ava, inspected Aug. 3 for Mtn. View Food Truck Festival. No violations.

Outlaw Charpit mobile food truck, Mtn. View, preopening inspection conducted Aug. 21. Reinspection prior to opening scheduled for Sept. 23. Four violations: wood shelving not painted or sealed for ease of cleaning, no designated hand-washing sink or three-compartment sink for dishwashing, windows without screens to prevent insects from coming in, and damaged floors and walls in disrepair, presenting a challenge for cleaning. Must be corrected by reinspection.

Ozarks Healthcare Foundation Coffee Shop, 1100 Kentucky Ave., inspected Aug. 2. One violation: prep hand sink hot water registered 83 degrees instead of the minimum 100. Business committed to repairing or replacing the faucet.

Ozarks Healthcare Grill 59, 1100 Kentucky Ave., inspected Aug. 2. Three violations: wiping cloths not stored in sanitizer when not in use, food boxes on floor in walk-in cooler and freezer, and prep area open front cooler registered 47 degrees, above the maximum of 41 degrees. All three items were corrected onsite.

Ozarks Sweetest Bakeshop, 704 E. Main St., Willow Springs, inspected Aug. 29. Three violations: hair restraints not worn by all food preparation staff, wiping cloths not stored in sanitizer while not in use and observed three-vat sink used for hand washing. New sink will be placed for hand washing before next routine inspection. First two items corrected onsite.

Pizza Americana, 210 E. Main St., Willow Springs, inspected Aug. 29. One violation: unlabeled salad bar items were observed. Staff said they would label them.

Pizza Inn, 1805 Bruce Smith Parkway, inspected Aug. 1. Will reinspect for follow-up Sept. 3. Two violations: excessive ice accumulation in walk-in freezer in need of repair and mouse droppings observed in back storage room and at server area soda fountain near game room. Business instructed to have professional pest control onsite to address the mouse problem, save the receipt and seal any outer openings.

Quality Inn, 220 Jan Howard Expressway, West Plains, inspected Aug. 6. Follow-up inspection scheduled for Aug. 27. Of 10 rooms inspected, issues were noticed with six: Room 103 had trash under the bed and smoke detector was not working; room 108, smoke detector not working; room 110, cobwebs in corner near chair; room 203, trash under bed and smoke detector not working; room 314, entryway and bathroom floors dirty, trash under bad; and room 316, roach on wall near microwave and trash under bed. Those issues accounted for four violations. Five additional violations were noted: paper towels needed at prep area hand sink in kitchen, emergency light in south stairwell near ice machine was not working, second floor laundry chute door was open and latches were not operational on the second or third floor chutes, no chlorine available when pool was tested and sediment in pool in need of cleaning.

Ramey’s Supermarket No. 9, 1211 Parkway Center, West Plains, follow-up inspection conducted July 11. Three violations: produce shelving in walk-in cooler rusty and in need of replacement, unshielded light bulbs in deli prep and display coolers and back loading bay door open. The latter was corrected onsite. The remaining issues must be corrected by the next routine inspection.

Ruby Tuesday, 1008 Worley Drive, West Plains, inspected Aug. 5. Follow-up inspection scheduled for Nov. 12. Four violations: foods stored uncovered in prep coolers and freezers; mashed potatoes at steam table registered 125 degrees, below the minimum 135; floor tiles in prep and storage areas broken and in need of replacement or repair; and excessive buildup in need of cleaning at the dishwasher wall/floor, behind prep table coolers, at the grill and over the flooring, and flooring in the walk-in cooler.

San Juaneros, 310 Kentucky Ave., Suite 2, inspected Aug. 7. Three violations: food stored uncovered in prep coolers, paper towels needed at prep area hand sink and broken or missing tiles in need of removal or replacement in prep/dish areas. The first two items were corrected onsite and the last must be corrected prior to the next routine inspection.

Four violations: hot dogs on roller registered 110 degrees and hamburgers in warmer, 114 degrees, both below the minimum of 135; soda fountain area in back in need of cleaning and repair, plus replacement of ice machine covers; no hand washing signage for employees in restroom; and unlabeled spray bottle found in back storage area. Deadline of July 30 given for corrections for all but the temperature issue, which was given a deadline of July 11.

Snappy Mart No. 9, 1671 U.S. 63, West Plains. Inspected Aug. 28. Follow-up inspection scheduled for Oct. 30. Four violations: chest freezer with ice accumulation in need of defrosting, soda fountain and heads in need of cleaning, no paper towel dispenser at prep area handsink and damaged flooring in prep and service area in need of repair. All to be corrected by reinspection.

Sonic, 1710 Gibson Ave., West Plains. Follow-up inspection conducted July 12. Previous violations corrected. No new violations.

Spring Dipper, 675 Missouri Ave., West Plains, inspected Aug. 6. One violation: outside freezer and second dipping well in need of defrosting. Business committed to correcting that week.

Steak N Shake, 989 Worley Drive, West Plains, inspected Aug. 1. Three violations: Chili in warmer was 103 degrees, below minimum 135; employee hand washing sign had been removed from restroom; and mold observed on dish area walls and cobwebs seen in soda syrup storage area in need of cleaning. Chili and cleaning issues were corrected onsite, and staff committed to ordering new hand washing signage.

Subway of Willow Springs, 857 E. Main St., Willow Springs, inspected July 15. Two violations: Hot water at front hand sink was 97 degrees, below the minimum of 100, and water damaged ceiling tiles in back area were in need of replacement. The first issue was corrected onsite with an adjustment, and the business committed to replacing the tiles as soon as possible.

West Plains Quickstop No. 2, 7198 U.S. 160, West Plains. Follow-up inspection conducted Aug. 1. Will reinspect Sept. 10; must have noted violations corrected at that time. Five violations: hair restraints not worn by all food prep staff; food prep employee preparing burgers with bare hands; gravy on warmer measured 118 degrees, below the minimum of 135 degrees; eggs seen sitting out at room temperature instead of in cooler; and thermometers not seen in all cold holding units.
